Apply for a Pharmacy Benefits Manager (PBM) License in Missouri

You must satisfy these requirements:

Submit a completed application form online and pay a one thousand dollar ($1,000) application fee. A Pharmacy Benefits Manager license shall be effective for one year from the date it is issued.

Note: All Pharmacy Benefits Managers must notify the department of any changes to the information supplied in the application within sixty (60) calendar days of the date of the change.
