- Michael R. Dicken, Poplar Bluff, MO, insurance agent license revoked for practicing or aiding/abetting fraud, forgery, deception, collusion or conspiracy in an insurance transaction and demonstrating lack of trustworthiness or competence.
- Denise A. McDonald, Streator, IL, application for license denied because of criminal history.
- Duane Miller, Duluth, MN, license as bail bond agent revoked for incompetence, misconduct, gross negligence, fraud, misrepresentation and dishonesty. Respondent did not appear for hearing.

- USAA Insurance Cos., San Antonio, TX, $13,188 forfeiture for failure to use required language on after-market parts on auto estimates; underpaying private passenger auto medical and total-loss claims; failure to maintain documentation of medical bills to determine if paid accurately and reasonably; not allowing homeowner claimants to recover replacement cost with statutory 10 years; and failure to make timely payment of differnece tween actual cash and replacement value on homeowners loss in excess of $10,000.
