1. Am I part of the settlement?
    Anthem, as required by the settlement, will identify and contact policyholders covered under the settlement and will return premiums to those covered under the settlement.

  2. Is there anyone I can contact to find out if I’m part of the settlement?
    Consumers can contact Anthem directly at 877-889-1954 or the number listed on the back of their insurance ID card.

  3. What should I do if I believe I should be part of the settlement and Anthem never contacts me?
    Consumers who believe they should be part of the settlement, but have not been contacted by Anthem should contact the department’s Consumer Hotline at 800-726-7390.
