Missouri residents and those with Missouri insurance policies may file complaints directly with the Missouri Department of Insurance, Financial Institutions & Professional Registration.
File your complaint on-line or by paper through mail or fax.
If you feel you are the victim of fraud, a Fraud Report may be filed as well.
Physicians and Doctor’s Offices, if you wish to file a complaint related to prompt payment of health insurance claims, please complete a Provider Complaint Form and submit by fax to the number below or mail to address indicated on the form.
